Immunization Requirements

2006-2007

In accordance to Texas State Law, students will not be allowed to attend class if his/her immunization records are not presented to the school upon enrollment and if the student's immunization records are not current. The following immunizations are required for all Texas students:

PPCD/Speech (3 years of Age)

Four doses of DPT/DTaP or DT if Pertussis is medically contraindicated

Three doses of Polio (IPV) or if any combination of 4 doses of OPV and IPV was received before 4 years of age, no additional dose is required.

One dose of MMR, on or after the 1st birthday

Three doses of Hib, (a primary series with one dose on or after 1st birthday), or 1 Hib on or after 15 months

Three doses of Hepatitis B

One dose of Varicella, on or after 1st birthday, or parent/physician statement of chickenpox illness

One dose of Pneumococcal Conjugate Vaccine on or after 24 months unless a series has been completed.

Two doses of Hepatitis A (6 to 18 months apart) the first dose received on or after 12 months of age.

Pre-Kindergarten (4 years of Age)

Four doses of DPT/DTaP/ or DT if Pertussis is medically contraindicated

Three doses of Polio (IPV) or if any combination of 4 doses of OPV and IPV was received before 4 years of age, no additional dose is required.

One dose of MMR, on or after the 1st birthday

Three doses of Hib, (a primary series with one dose on or after 1st birthday), or 1 Hib on or after 15 months

Three doses of Hepatitis B

One dose of Varicella, on or after 1st birthday, or parent/physician statement of chickenpox illness

One dose of Pneumococcal Conjugate Vaccine on or after 24 months unless a series has been completed.

Two doses of Hepatitis A (6 to 18 months apart) the first dose received on or after 12 months of age.

KG  Twelfth Grade

DPT/DTap - Five doses for children 4 to 6 years; if the fourth dose was given on or after the 4th birthday, then a 5thdose is not necessary. Three doses are required for children 7 years and older with at least one dose on or after the 4th birthday.

TD Booster is required every 10 years after last dose of DTP/DTaP/DT

Four doses of Polio (IPV), one since the 4th birthday, or if 3rd dose was given on or after 4th birthday, only

three doses required; 5th dose is required if 4th dose was given before 4th birthday or

If any combination of 4 doses of OPV and IPV was received before 4 years of age, no additional dose is required.

Polio Vaccine is not required for students 18 years and older.

Two doses of MMR with the 1st dose on or after the 1st birthday; 2nd dose - at least 28 days apart from the 1st Three doses of Hepatitis B

One dose of Varicella, on or after 1st birthday, or parent/physician statement of chickenpox illness;

two doses of Varicella vaccine are given if 13 years of age or older

TB skin test  recommended but not required for all grade levels.

Hepatitis A  A series of two doses is recommended but not required for students in grades K-12.

Vaccines administered after September 1, 1991, shall include the month, day, and year each vaccine was administered.

Receipt of the dose up to (and including) 4 days before the birthday will satisfy the school entry immunization requirements

Exclusion from compliance includes medical reasons (a statement from a M.D. or D.O. is required), stating that in the physician's opinion that the vaccine required is medically contraindicated or poses a significant risk to the health and well-being of the child. Unless it is written in the statement that a life-long condition exists, the exemption statement is valid for only one year from the date signed by the physician. Conscientious Exemptions, including religious belief, must be obtained from the Department of State Health Services and submitted to the school nurse for immunization exemption upon enrollment. It must be renewed every 2 years.

Provisional Enrollment: A student may be provisionally enrolled if he/she has begun the required vaccine series, and is receiving subsequent immunizations as rapidly as medically feasible. If a student is transferring from another Texas school he/she may be enrolled for 30 days while awaiting the transfer of the immunization record. Children of military families who are moving to Texas from out-of-state and awaiting the transfer of immunization records may be provisionally enrolled for 30 days. A student who is homeless, as defined by the McKinney Act, shall be admitted temporarily for 30 days if acceptable evidence of vaccination is not available.
