TO: ALL INCOMING 7^TH, 9^TH, & 11^th GRADE ATHLETES

SUBJECT: PHYSICALS

All incoming 7^th, 9^th, and 11^{th grade athletes as well as any new athletes that plan to play sports, cheerleading, or dance are required to have a physical before  they are allowed to try out or compete in athletics.}

^{We have made the following arrangements for physicals to be given at school by Dr. Rogers and Dr. Veselka of Family Practice Associates.  The cost will be $20.00 per student, please bring cash or check made payable to Family Practice Associates.} 

^{YOU MUST BRING YOUR MONEY ON THE SPECIFIED DATE}

^{8th Grade:    Wednesday, May 9, 2007                   
Starting at 7:40 a.m. & Athletic  Period.}

^{6th Grade:    Thursday, May 10, 2007                    
Starting at 8:30 a.m.}

^{10th Grade: (going to 11th)  Wednesday, May 16, 2007                     
Boys (athletic period)
Girls (athletic period)}
